When does a deck need a railing?

When it is more than 30 inches above grade: the International Residential Code, as laid out in the American Wood Council's DCA 6 deck guide, requires a guard on those decks. Lower decks may not need one, but any railing that is there should still be solid.

Why is my deck railing wobbly?

Usually because the posts are loose at the deck frame: bolts back out, wood shrinks, or the post or joist rots where they meet. The DCA 6 guide calls for guard posts of at least 4x4 attached to joists of at least 2x8, so small posts on thin framing are a common cause.
